ABSTRACT

Purpose Blast lung injury is severe, and is the main causes of death in explosion. This paper aims to explore the imaging features of blast lung injury, and to provide reference for clinical diagnosis and care. Materials and Methods The clinical data of 37 patients with primary blast lung injury stricken in the 8.2 Kunshan explosion in 2014 were retrospectively studied, with regard to the degree of burn, area, auxiliary emergency treatment and medical history. The imaging manifestations of X-ray and/or CT were also analyzed. Results ① The X-ray and/or CT findings of chest showed that 6 cases (16.2%) presented no obvious abnormality of lung parenchyma, 14 cases (37.8%) manifested increase of lung markings and spot-like clouding opacity, 6 cases (16.2%) presented ground glass opacity, 7 cases (18.9%) manifested patchy shadow, 4 cases (10.8%) showed diffuse consolidation. 6 cases (16.2%) with pleural effusion, 6 cases (16.2%) complicated with pneumothorax and hydropneumothorax, 4 cases (10.8%) with enlarged podoid and pericardial effusion, 2 cases (5.4%) with mediastinal hematoma, 9 cases (24.3%) with rib fracture. The positive rate of X-ray film inspection was 72.2% (13/18), yet that of CT reached 92.9% (13/14). ② Five patients who had not been seen obvious abnormal in emergency lung examination appeared mottling shadows or flake fuzzy shadows in the examination on the second day; 9 cases with aggravated symptoms and infection signs displayed relieved signs, expanded or changed primary lesions. Conclusion Imaging examination is an important tool in the diagnosis of primary blast lung injury. CT is superior to X-ray film on the detection of lesions; therefore, CT examination on chest should be carried out as soon as possible if conditions are allowed.

ABSTRACT

Objective To explore the clinical value of 256-slice CT whole hepatic perfusion imaging in transcatheter arterial chemoembolization (TACE) for hepatocellular carcinoma (HCC).Methods Twenty-three patients with hepatocellular carcinoma underwent whole hepatic perfusion with the JOG technique one week before TACE.The scanning data of cancer and liver tissues were analyzed using the perfusion software. The cancer tissue perfusion was repeated 4 to 6 weeks after treatment and was compared with that before treatment. Results (1) Before TACE,the HCC lesions were shown on the hepatic arterial perfusion (HAP) map as homogeneous hyper-perfusion lesions in 8 patients and as inhomogeneous hyper-perfusion lesions in 15 patients. The HAP and hepatic arterial perfusion index (HAPI) values of the tumor were higher than those of the liver tissues, while the hepatic portal perfusion (HPP) values of the tumor was lower than that of the liver tissues. The differences were all significant (P < 0.05).(2) Mter TACE,the tumors were totally filled with lipiodol in 3 patients, partially filled with lipiodol in 13 patients, and sparsely filled with lipiodol in the remaining 3 patients. There was no blood perfusion in the lipiodol-filled areas and in the cancer necrotic tissues, but in the sparsely or partially lipiodol-filled areas blood perfusion could still be seen.21 patients received another session of TACE 6 to 8 weeks later. The results between the hepatic arterial digital subtraction angiography (DSA) and the CT perfusion were 100% matching. The HAP and HAPI values of the tumor decreased when compared with before treatment, and the difference was significant (both P <0.05).The HPP values decreased slightly with no significant difference (P > 0.05),while higher HAP and HPI and lower HPP were observed in the active cancer tissues when compared with the liver tissues after treatment (P < 0.05).Conclusion 256-slice MSCT whole liver perfusion imaging can quantitatively reflect abnormal perfusion of hepatocellular carcinoma tissues and postoperative active tissues, and has important guiding significance in the preoperative evaluation, and the postoperative follow up of patients treated with TACE.

ABSTRACT

The effects of hypoxia on the expression of Fos was studied in the distal cerebrospinal fluid contacting neurons (CSF-CNs) in the dorsal raphe nucleus (DR) of the rat. The hypoxic models mimic 8000 m high level were established in a high-altitude decompression chamber. Injecting the tracer of CB ( chorela toxin B subunit) into the rat's lateral ventricles, we investigated the distribution of the distal CSF-CNs in the DR. The single and double immunohistochemistry staining with antibodies to Fos and CB were used to observe the expression of Fos in the distal CSF-CNs in the DR. The results showed that a large number of CB-like immunoreactive (-LI) neurons (31.16 ±3.36/per section) were located in the DR. In hypoxic experimental rats, the number of Fos-LI neurons was increased sharply (40.28 ±2.17/per section, P <0.05 compared to control rats), and a few CB/Fos-LI neurons (2.00 ±0.39/per section) could be observed in the DR. In control rats, several Fos-LI neurons (5.55 ±0.81/per section) and no CB/Fos-LI neuron could be observed in the DR. These results suggest that some of the distal CSF-CNs in the DR may play roles in transmitting information between brain and cerebrospinal fluid and modulating the function of brain following hypoxic stimulation.

ABSTRACT

Objective In the present study, we observed the distribution of nitric oxide synthase positive neurons in the developing rat telencephalon and to realize the possible roles of NO in brain development further. Methods NADPH-d histochemical method was used to investigate the micromorphology and distribution of NOS positive neurons in rat telencephalon from embryonic day 14 (El4) to postnatal day 14 (P14). Results At El4, no neurons stained. Labeling was observed in the ventrolateral of the striatum at E15. At E17, positive neurons were seen in the cerebral cortex and piriform cortex. They were cells with small cell bodies, shorter dendrites and less ramification. The stained neurons were either increased or maintained in both number of neurons and intensity during development. At E20, positive cells were found in many regions. Many positive neurons were present in the piriform cortex, ventrolateral of the striatum and bed nucleus of the stria terminalis. They acquired larger size and more complex dendrites. After birth, intensely stained NOS positive fibers were distributed widely in the cerebral cortex and striatum. Stained cells were found in the corpus callosum and hippocampus also. The distribution pattern of positive cells at P14 was essentially similar to that reported in adult rats. Conclusion The distinctive temporal and spatial expression pattern of nitric oxide synthase positive neurons in the telencephalon indicated that NO may play an important role in brain development and maturation.

ABSTRACT

Objective To observe the activation of astrocytes, microglia and expression of proinflammatory cytokines in the periaqueductal gray(PAG) by Complete Freunds Adjuvant(CFA)-induced inflammatory pain. Methods Heat hyperalgesia and mechanical allodynia were observed and the expressions of mRNA,protein of glial markers, and proinflammatory cytokines in the PAG were examined following intraplantar administration CFA in right hindpaw of the rat by behavioral testing, reverse transcription polymerase chain reaction(RT-PCR), Western blotting, and immunohistochemistry. Results CFA injection induced chronic thermal hyperalgesia and mechanical allodynia. After injection, the thermal hyperalgesia was recovered on Day 14,but the mechanical allodynia lasted more than 21 days.Microglial marker(Cluster of differentiation 14,CD14) and proinflammatory cytokines(Interleukinin-1?,tumor necrosis factor-?) showed a significant increase in their expression during all phases(acute, subacute and chronic) of inflammation.GFAP was observed only in PAG at subacute and chronic phases of inflammation. Conclusion The activation of microglia and astrocytes may be involved in the initiation and maintenance of inflammatory pain.Enhanced cytokines expression might play a role in eliciting behavioral hypersensitivity.

ABSTRACT

Objective To investigate the neuroprotective effect of inducible nitric oxide synthase(iNOS)inhibitor, aminoguanidine(AG),after brain injury in rats. Methods Using frontal sharp cortex injury model,SD rats were randomly divided into AG treat group and NS control group.Biochemical assay,RT-PCR,immunohistochemistry and TUNEL method were used to determine NO content,iNOSmRNA,iNOS positive cells and apoptotic cells respectively. The experimental results were compared in the two groups. Results NO content and the expression of iNOSmRNA and iNOS positive cells in traumatized cortex decreased significantly in AG treat group than that in NS control group. The number of apoptotic cells also reduced correspondently in AG treat group. Conclusion AG could selectively inhibit the expression of iNOS,then reduce NO content, thus reduce apoptotic cells and produce protective effect on cells in injuryed area.
